The launch of NCCARE360 – the nation’s first state-sponsored technology platform designed to connect healthcare providers and human services organizations – formally began last January, with full statewide implementation targeted to be complete by the end of 2020. As of June 30th, more than 250 organizations across 12 active counties on now onboarded with the program. To date, the program has generated over 400 referrals, impacting 216 people. Navigating services like health, housing, employment, and transportation can be incredibly difficult, but all are factors that contribute to an individual’s overall health. NCCARE360 provides public access to resources designed to help health and community-based organizations make electronic referrals, communicate in real time, securely share client information, and track outcomes together.
