NC DMA will be linking more Medicaid beneficiaries with a CCNC primary care provider throughout this fiscal year for the state.

Patients will be linked using an automated and manual process to a CCNC medical home with whom they have an existing relationship or to one shown on file as having open enrollment for new patients. Patients who have been assigned to a medical home and present with questions, should be directed to call DMA toll free at (855) 843-3402.
