2010 Enrollment in the State Health Plan will begin mid-March and extend through April. Each member is being subjected to a benefit differential based on tobacco use or tobacco cessation program participation. Plan members who state that they or their covered dependents are participating in a tobacco cessation program, must submit a physician-verified participation form. A copy of this form, as well as background information published by the Plan, has been posted on the NCAFP website.
