CCNC Improves Health Care Quality
CCNC is the most successful large scale implementation of a patient-centered and physician-directed healthcare delivery system. The program's integrated design enables it to achieve healthcare performance that bests 90% of all commercial Medicaid managed care organizations on quality measures related to chronic diseases such as diabetes, heart disease and asthma. This high quality care drives cost savings in the hundreds of millions of dollars annually.
Exceptional Quality & Performance
- All 14 CCNC Networks participate in quality initiatives aimed at improving quality in asthma, diabetes, heart disease, urgent care, and case management.
- CCNC has developed unique quality performance incentives for asthma and diabetes to motivate and incentivize physician/practice adoption.
- Since 2002, CCNC has been a leader in developing practical cost containment mechanisms related to pharmacy and prescription drug costs. Its Prescription Advantage List has helped save money and improve quality.
- North Carolina has been featured as a 'best practice state' by the Agency of Healthcare Research and Quality as a result of CCNC's work.
- CCNC manages approximately 990K patients through the personalized, medical home model.
- The program's 14 networks serve all 100 NC counties, but are managed locally, with local residents and professionals.
- CCNC embraces and executes enhanced Primary Care Case Management that lowers costs and improves quality.
- All CCNC networks participate in program-wide quality improvement initiatives, but are also free to pursue/establish their own network-specific quality programs.
Patient-Centered, High-Touch Healthcare
- Each patient (990K) is assigned their own medical home and primary care provider. Care is coordinated through this provider/practice.
- CCNC Case Managers identify patients with high risk conditions or needs, assisting the providers in disease management education. This lowers costs and improves quality.
- Case Managers assume different responsibilities depending on the community and providers' needs.
- CCNC has developed a custom Case Management Information System (CMIS) that is utilized statewide. The technology enables collaboration, data access, case assignment and other functions.
